Please find below the forms required to apply for Clinical Accreditation at The Sunshine Coast Private Hospital. Clinical Accreditation is required for all Visiting Medical/Dental and Allied Health Practitioners including Surgical Assistants and Dental Assistants.
The application forms detail the supporting documentation that is required to accompany your application. Prior to processing your application the hospital is required to receive ALL doccumentation. You are also required to read and acknowledge the UnitingCare Health By-Laws and Code of Conduct as part of your application.
